If you –– or someone you — know have lost health insurance and/or have struggled to put food on the table as a result of the COVID-19 pandemic, complete this form to determine your eligibility for Medi-Cal, CalFresh, and/or CalWorks — three social service programs for low-income individuals in California.
After completing the form, a Community Wellbeing San Diego representative will contact you within 2-4 days to inform you of your eligibility as well as process your application if you qualify.
